IN APPLYING FOR CERTIFICATION AS A WETLAND PROFESSIONAL (PWS or WPIT),
THE FOLLOWING MUST BE SUBMITTED:

a) Original application plus three complete photocopies,
b) Transcript(s) plus three photocopies,
c) Three sealed letters of reference and the
d) Application fee

Professional Certification Program Application Checklist

1. Is the application filled out completely, including the References section?
2. Is Form B signed, dated and enclosed?
3. Have you enclosed the original application and three full photocopies (sealed references to be photocopied by the business office only)?
4. Are the transcripts stapled to each application?
5. Are the three sealed letters of reference included?
6. Is the appropriate application fee enclosed?
7. Have you made copies of all materials for your records?
